Physical Activity Among Postoperative Esophageal Cancer Patients

BACKGROUND: Previous literature has reported that regular physical activity enhances health-related quality of life for cancer patients. However, there is a lack of studies that focus on physical activity among postoperative esophageal cancer patients. OBJECTIVE: The aims of this study were to (1) describe the prevalence of physical activity among postoperative esophageal cancer patients, (2) explore variables related to physical activity (demographics, nutrition, dysphagia, and health-related qualify of life), and (3) examine potential reasons for inactivity among participants who scored low using case studies. METHODS: In this cross-sectional design, physical activity was evaluated by the Japanese version of the International Physical Activity Questionnaire. Variables were analyzed using Spearman rank correlation coefficients, Mann-Whitney U tests, or Fisher exact tests. RESULTS: Fifty-eight patients participated in this study; 79% met the recommended physical activity guidelines. Present occupational status and past leisure-time physical activity behavior before the cancer diagnosis were related to current leisure-time physical activity and meeting the guidelines postoperatively. Participants who scored 0 (little or no activity) in the questionnaire also scored low in health-related quality of life. CONCLUSIONS: Participants performed especially well in physical activity related to leisure time and transportation. Positive reinforcement is needed for patients who perform adequate levels of physical activity, and reviewing the benefits of regular physical activity is encouraged for all. IMPLICATIONS FOR PRACTICE: Occupational status and past leisure-time physical activity before diagnosis can be indicators for promoting physical activity among postoperative esophageal cancer patients. Inactive participants should be supported to promote both physical activity and health-related qualify of life.

高齢者地域コミュニティのグループ運動継続に関わる特徴

目的:高齢者地域コミュニティのグループ運動継続に関わる特徴を把握する。事業/活動内容:対象は藤沢市で実施中の身体活動促進のための地域介入研究に参加し、6ヵ月間グループ運動を継続している8グループ(120名)である。研究参加時、6ヵ月後時点で質問紙調査、体力測定、認知機能評価を行った。運動プログラムやモニタリングツールを提供し、定期的なフォローアップや交流会、グループワークを行った。事業/活動評価:(1)リーダーの特徴と協力者の有無および協力の主体性で8グループを4群に分類し、質問紙による身体活動量、精神的健康、地域とのつながり、個人要因を比較した。(2)グループ向け質問紙、交流会、グループワークの質的情報から、グループの特徴を比較した。結果を統合してコミュニティづくりに重要なルール(自生した規則や制度)、ロール(自発的な役割)、ツール(道具や資源)の観点からグループの特徴を把握した。活動基盤となるルールは、全グループ共通のルール、円滑なグループ活動を行うためのルール、運動実施・継続のためのルールの3つに分類できた。今後の課題:ルールが明確なグループには、ロールがあり、効果的にツールを活用できていることが示唆された。住民参加型の身体活動促進の取り組みを通して、グループ運動実施・継続のためのルール、ロール、ツールが把握できた。長期的観察と成果の活用に向けた取り組みが今後の課題である。(著者抄録)

「健康と生活に関するアンケート」に回答した高齢者534名(男性260名、女性274名、68〜78歳)を対象とした。男女の比較で有意差を認めた人口統計学的属性・社会経済的属性は、居住形態(女性の独居割合が高い)、BMI(男性が高い)、身体活動の制限(女性の医師または足腰の痛みによる制限がある割合が高い)、教育歴(男性の大学卒以上の割合が高い)、収入がある仕事(男性の仕事をもつ割合が高い)であった。ソーシャル・キャピタル(SC)では地域の人とのかかわりと他世代との交流において有意な男女差(女性のSCが高い)を認めた。SCが低いと想定される回答を基準とした解析で、男性では、地域の支え合い:支え合うと思う、地域の人とのかかわり:立ち話をする人がいる、相談しあえる人がいる、他世代との交流:まあまあある、とてもあるにおいて有意な正の関連を認めた。トレンド検定では、有意な量反応関係を認めた。女性では、地域の人とのかかわり:相談しあえる人がいる、地域組織活動への参加:1団体以上において有意な正の関連を認めた。トレンド検定では、「地域の人とのかかわり」において有意な量反応関係を認めた。

本研究の目的は、日本人高齢者を対象に「アクティガイド」や「プラス・テン」などの身体活動プロモーションの認知と身体活動時間、座位行動時間の関連について検討することである。我々は2013年より神奈川県藤沢市において身体活動促進のためのコミュニティワイドキャンペーンを実施している。この取り組みの成果評価及び2015年度以降に行う全市的な介入の事前評価の位置づけで、2015年6月に20歳以上の藤沢市民3,000名を無作為抽出して質問紙調査を実施した。本研究ではその質問紙調査を用い、65歳以上の高齢者874名を対象者とした。このうち550名から返答があり、データに欠損のない373名を解析に用いた。48%(179/373名)が「アクティブガイド」や「プラス・テン」などの身体活動プロモーションを認知していた。身体活動時間(運動を行っている時間と歩行またはそれと同等以上の強度の生活活動を行っている時間の総和)の中央値は100分/日、座位行動時間の中央値は300分/日であった

Association of high individual-level of social capital with increased physical activity among community-dwelling elderly men and women: A cross-sectional study

We aimed to evaluate the associations of individual-level social capital (SC) and physical activity (PA) among Japanese elderly men and women. Individuals aged 65 or above were selected via population-based random sampling of 3000 adults, aged 20 years or above and living in Fujisawa city, who were asked to complete self-administered questionnaires. The sample consisted of 260 men [median (25-75% tile) age 73 (68-78) yrs] and 274 women [73 (68-78) yrs]. The daily duration of PA, SC [e.g., norms of reciprocity (mutual support in the community), social networks (relationship with community people, exchange with other generations), social participation (participation in community organization activities)], socio-economic status [educational attainment, work with income, perceived household economic status], and demographic characteristics [sex, age, living arrangements, BMI, physical limitations] were assessed. Multivariate logistic regression analyses were used to calculate odds ratios (OR) for associations of SC with PA, stratified by sex. High level of mutual support in the community, relationship with community people, and exchange with other generations were associated with longer PA time in men after adjusting for potential confounders in the final model (OR = 2.40 (95% CI: 1.24-4.67), 5.28 (1.42-19.66), and 4.24 (1.15-15.67), respectively). In women, relationship with community people and participation in community organization activities were associated with longer PA time (3.10 (1.32-7.26) and 2.50 (1.27-4.91), respectively). The association of individual-level SC and PA differed between men and women. Future studies can investigate whether similar results can be obtained in other areas or attempt to conduct a large-scale survey.

© 2017 Elsevier Inc. Very few community intervention studies that promote physical activity (PA) using guidelines and its dissemination and implementation have been conducted. Consequently, we evaluated the effectiveness of a community-wide intervention (CWI) of PA with adults based on the Japanese guidelines for promoting PA. This was a non-randomized controlled trial, with four administrative districts in Fujisawa city assigned to the intervention group and nine to the control group. The CWI, conducted from 2013 to 2015, comprised information dissemination, education, and community support. The primary outcome was change in PA participation. Secondary outcomes were CWI awareness and PA guideline knowledge. Outcomes were assessed using questionnaires distributed to two independent, random samples of 3000 community-based adults (aged ≥ 20 years). Two separate samples—1230 adults at baseline and 1393 at the two-year follow-up—responded to the survey. The median time spent in PA did not differ between intervention and control groups after adjusting for potential confounders (adjusted difference between groups = − 0.02 min/day [95% confidence interval (CI): − 0.11, 0.10]). However, intervention group participants were more aware of the CWI (33.8%) than were control group participants (25.2%) at the two-year follow-up (odds ratio = 1.44 [95% CI: 1.06, 1.95]). A significant difference was also observed in participants’ PA guideline knowledge (adjusted difference between groups = 0.82% [95% CI: 0.33, 1.31]). Although significant differences in awareness and knowledge were observed between groups, this CWI did not change PA levels over two years. Future studies should investigate the long-term effects of CWIs beyond two years. Trial registration number: UMIN-CTR UMIN000018389.

Regular group exercise contributes to balanced health in older adults in Japan: a qualitative study

BACKGROUND: While community-wide interventions to promote physical activity have been encouraged in older adults, evidence of their effectiveness remains limited. We conducted a qualitative study among older adults participating in regular group exercise to understand their perceptions of the physical, mental, and social changes they underwent as a result of the physical activity. METHODS: We conducted a qualitative study with purposeful sampling to explore the experiences of older adults who participated in regular group exercise as part of a community-wide physical activity intervention. Four focus group interviews were conducted between April and June of 2016 at community halls in Fujisawa City. The participants in the focus group interviews were 26 older adults with a mean age of 74.69 years (range: 66-86). The interviews were analysed using the constant comparative method in the grounded theory approach. We used qualitative research software NVivo10(R) to track the coding and manage the data. RESULTS: The finding 'regular group exercise contributes to balanced health in older adults' emerged as an overarching theme with seven categories (regular group exercise, functional health, active mind, enjoyment, social connectedness, mutual support, and expanding communities). Although the participants perceived that they were aging physically and cognitively, the regular group exercise helped them to improve or maintain their functional health and enjoy their lives. They felt socially connected and experienced a sense of security in the community through caring for others and supporting each other. As the older adults began to seek value beyond individuals, they gradually expanded their communities beyond geographical and generational boundaries. CONCLUSIONS: The participants achieved balanced health in the physical, mental, and social domains through regular group exercise as part of a community-wide physical activity intervention and contributed to expanding communities through social connectedness and mutual support. Health promotion through physical activity is being increasingly emphasized. The study results can help to develop effective physical activity programs for older adults in the community.

Validation of Questionnaire-Assessed Physical Activity in Comparison With Objective Measures Using Accelerometers and Physical Performance Measures Among Community-Dwelling Adults Aged >/=85 Years in Tokyo, Japan

BACKGROUND: To date, there is no physical activity (PA) questionnaire with convergent and construct validity for the oldest-old. The aim of the current study was to investigate the validity of questionnaire-assessed PA in comparison with objective measures determined by uniaxial and triaxial accelerometers and physical performance measures in the oldest-old. METHODS: Participants were 155 elderly (mean age 90 years) who were examined at the university and agreed to wear an accelerometer for 7 days in the 3-year-follow-up survey of the Tokyo Oldest-Old Survey of Total Health. Fifty-nine participants wore a uniaxial and triaxial accelerometer simultaneously. Self-rated walking, exercise, and household PA were measured using a modified Zutphen PA Questionnaire (PAQ). Several physical performance tests were done, and the associations among PAQ, accelerometer-assessed PA, and physical performances were compared by Spearman's correlation coefficients. RESULTS: Significant, low to moderate correlations between PA measures were seen on questionnaire and accelerometer assessments (rho = 0.19 to 0.34). Questionnaireassessed PA measure were correlated with a range of lower extremity performance (rho = 0.21 to 0.29). CONCLUSIONS: This PAQ demonstrated convergent and construct validity. Our findings suggest that the PAQ can reasonably be used in this oldest-old population to rank their PA level.

Relationships of Muscle Echo Intensity with Walking Ability and Physical Activity in the Very Old Population

This study investigated the relationships between muscle echo intensity (EI), physical activity (PA), and functional mobility in 108 Japanese (88-92 years). We measured EI and muscle thickness (MT) at the midpoint of the anterior superior iliac spine and patella using B-mode ultrasound. Light and moderate-to-vigorous PA (LPA and MVPA) were assessed with a triaxial accelerometer. The timed up and go (TUG) test was used to measure for functional mobility. EI, but not MT, was significantly associated with both TUG scores (beta = 0.17, p =.047) and MVPA (beta = -0.31, p =.01) when adjusted by potential confounders. However, association between EI and TUG disappeared after adjusted for MVPA. Meanwhile, MVPA was significantly associated with TUG scores independent of EI (beta = -0.35, p <.001). Although EI of anterior thigh muscles might be a weaker predictor of functional morbidity than MVPA, it is a noninvasive and practical approach for assessing muscle quality in the very old.

Effects of stretching exercise with a stretching-support cushion on flexibility in over middle-aged women

BACKGROUND: Recently, stretching-support cushions (SC) have become available for use during stretching exercises in the seated position. However, the effects of a SC consisting of a 20-deg posterior support cushion on flexibility-related outcomes when combined with stretching exercises are not well understood. Here, a randomized controlled trial involving 32 healthy over-middle aged women (56.9 +/- 7.4 years old) was performed to investigate the additional effects of stretching program with SC on the flexibility compared with the identical program without SC. METHODS: The participants were randomly assigned to stretching exercises with SC (EX-SC, N.= 16) or the identical stretching exercises without SC (EX, N.= 16). We investigated the additional effects of using SC on flexibility by measuring the degree of hip abduction, pelvic tilt, and maximum forward reaching distance, sit and reach test, and passive torque of right hamstrings during passive knee extension before and after the single session of program. RESULTLTS: After performing the stretching exercises with (EX-SC group) and without (EX group) the SC, significantly change in pelvic tilt in the seated straddle split position was observed in the EX-SC group (21.0 +/- 28.9 to 33.1 +/- 24.0 deg) compared with the EX group (22.1 +/- 20.8 to 26.9 +/- 22.8 deg; P<0.05). For all other flexibility-related parameters, only the time effects were significant (P<0.05). CONCLUSIONS: Taken together, the present results suggest that use of a SC is beneficial for performing stretching exercises when seated to avoid a `rounded-back position'.

Relationships between Composition of Major Fatty Acids and Fat Distribution and Insulin Resistance in Japanese

Objective. The aim of this study was to evaluate the relationships between the composition of free fatty acids (FFAs) and metabolic parameters, including body fat distribution, in Japanese. Methods. The study subjects were 111 Japanese patients (54 males, 57 females). Metabolic parameters and visceral and subcutaneous fat areas as determined by CT scanning at the umbilical level were measured. Glucose tolerance test (GTT) was performed by administering 75 g glucose orally. Results. The percentage of linoleic acid (C18:2), the greatest constituent among FFAs, was negatively correlated with visceral fat area (r = -0.411, p < 0.0001), fasting glucose (r = -0.330, p < 0.0001), HbA1c (r = -0.231, p = 0.0146), and systolic blood pressure (r = -0.224, p = 0.0184). Linoleic acid percentage was also significantly negatively correlated with HOMA-IR (r = -0.416, p < 0.0001) by simple correlation. Based on the findings of OGTT, the 111 subjects were classified into three groups: 33 with normal glucose tolerance, 71 with impaired glucose tolerance (IGT), and 7 diabetic subjects. The percentage of serum linoleic acid in diabetic subjects was significantly lower than that in normal subjects. Conclusion. We conclude that serum linoleic acid level is negatively correlated with the accumulation of visceral fat in relation to a reduction of insulin resistance in Japanese subjects.
