慢性腎臓病(CKD)発症予防には生活習慣の修正による腎機能の回復が可能な早期腎機能障害の段階からの腎機能評価が必要であるが、現行の健診における早期腎機能障害の評価には限界がある。本研究では尿細管障害マーカーを含む複数の腎機能マーカーを用いてこれまで一般集団では十分に検討されてこなかった早期の腎機能障害を精緻に評価し、脳・心血管疾患発症などの複数の疾患アウトカムとの関連を検討することで、腎機能障害に関する多面的なリスク評価を行う。さらに遺伝要因の影響を含めた詳細な規定要因の検証を行い、個人の体質を考慮した次世代型の健診・保健指導導入のためのエビデンスを創出する。

本研究は、開始後10年を経過した地域在住者コホート研究により、若年期から集積する心臓代謝リスクの併存状態（マルチモビディティ）からNCDsおよびフレイル・認知機能低下への移行を媒介する共通代謝経路・代謝バイオマーカーを疫学的に解明・検証した上で、その代謝経路と関連する変容可能な環境要因を明らかにして、中・高齢期の保健・介護予防一体的実施に最適な介入手法の基礎とすることを目的とする。また新型コロナウイルス感染によってその機能がリモデリングされる代謝経路にも焦点をあて、感染の後遺・遷延症状も考慮した地域保健現場での健康支援についても検討する。

一般住民における腎機能低下と無症候性脳血管障害の関連

近年、腎細小血管の解剖学的、機能的類似性から腎機能と無症候性脳血管疾患との関連が指摘され始めており、患者コホートからの報告ではその関連が報告されているが、一般住民からの報告はまだ少なく、その評価は分かれている。本研究では、40歳代から80歳代の850人の健常な日本人一般住民を対象として、腎機能およびその経年変化と無症候性脳血管疾患との関連、およびこの関連への循環器疾患発症因子の介在の程度を明らかにする。無症候性脳血管疾患は脳卒中および認知機能低下指標として注目されているため、本研究で得られた結果は、高齢社会における認知症・脳卒中の早期発見マーカーとして予防に繋がることが期待される。
本研究は一般地域住民から年齢層化無作為抽出した集団のうち協力が得られた者を対象として、頭部MR画像を用いて評価した無症候性脳血管障害と潜在性動脈硬化指標および危険因子の測定を実施している既存の前向きコホートの成績を用いて、腎機能およびその経年変化と無症候性脳血管疾患の関連を検討するものである。昨年度測定した随時尿中の微量アルブミン、クレアチニン、ナトリウム値をデータベースに突合し、日本人一般集団における尿アルブミン/クレアチニン比(ACR)およびeGFRの経年変化の分布を確認し、無症候性脳血管疾患との関連を検討した。
新型コロナウイルス感染拡大のため、成果発表の一部が遅延している。
次年度は最終年度であるため、日本人一般集団における無症候性脳血管疾患と尿アルブミン/クレアチニン比(ACR)およびeGFRの経年変化との関連についてさらに詳細に検討を進める。また、その成果を学会や専門誌で報告する予定である。

Association between dietary factors and cognitive impairment in Japanese population(Fostering Joint International Research)

This study aimed to identify related factors for low cognitive function in two groups of genetically similar Japanese men exposed to Westernized lifestyles from different starting points in life using cross-sectional data from Japanese-American based in Hawaii and the Japanese based in Japan. Cognitive function was evaluated by participants’ performance on the Cognitive Abilities Screening Instrument (CASI), a validated comprehensive measure of global cognitive function developed for use in cross-cultural and cross-national studies. Older Hawaii-based men were more likely to achieve a low CASI score than were older Japan-based men. In the present study, lower cognitive function among older Japanese men was mostly explained by the differences in educational attainment between the cohorts.

The aim of the project is to investigate association of cognitive ability which is defined by structural and functional assessment of brain with dietary habits. Measures of total intracranial volume, white matter hyperintensity volume and so on were obtained with magnetic resonance imaging. Assessments of functional cognitive abilities was used Cognitive Abilities Screening Instrument (CASI). Cross-sectional analysis suggested a positive association of vegetable intake with CASI score and a negative association of vegetable intake with white matter hyperintensity volume.

The aim of this study is to investigate the prevalence of subclinical atherosclerosis and the brain atrophy in the general population of Japanese women, and to clarify the relevant factors. Our target number of subjects of this study was 800. In 2015-2017, we have successfully conducted survey on 266 women, who were randomly sampled from local residents in Kusatsu city, Shiga. We have performed chest and abdominal computed tomography, head magnetic resonance imaging, carotid artery ultrasound, cognitive function test, physical function tests, blood pressure, blood and urine test, and electrocardiogram to the subjects. We will continue the survey on approximately 500 women additionally in 2018-2021.

Gut microbiota and sublinical atherosclerosis in general population

The aim of the present analysis is to investigate the association between gut microbiota and coronary artery calcification (CAC) in general Japanese men. Shiga Epidemiological Study of Subclinical Atherosclerosis (SESSA) is a prospective cohort study of randomly selected men from a general Japanese population. This analysis included 669 participants who completed a follow-up examination including analysis of gut microbiota. After adjustment for age, BMI, total cholesterol, HDL cholesterol, HbA1c, smoking, alcohol intake, regular exercise, and use of medication to treat hypertension, dyslipidemia or diabetes, frequency of Lactobacillus was associated with lower risks of coronary artery calcification (odd ratio 0.84 per 1SD [9%] increase, 95%CI 0.70-1.00) while there were no clear relationships for Bifidobacterium, Bacteroides, Prevotella or Clostridium. Lactobacillus appears to be associated with lower prevalence of CAC in a general population of Japanese men.

Explorative Study on new biomarker of atherosclerosis

We have reported the association of insulin resistance and progression of coronary artery calcification (CAC), and the association between step count and onset of metabolic syndrome in logitudinal analyses. We also reported a comparative study of CAC between our cohort (Shiga Epidemiological Study of Subclinical Atherosclerosis: SESSA) and a community-based cohort in the US (Multi-Ethnic Study of Atherosclerosis: MESA). In other collaborative studies with University of Pittsburgh, we have reported the association of n-3 fatty acid and CAC. Moreover, we investigated the association between lipoprotein-associated phospholipase A2 and atherosclerosis as well as the association of low density lipoprotein (LDL) cholesterol with polymorphism of aldehyde dehydrogenase. Metabolome in the urine, and oxidized LDL in the blood have been already measured and we will continue to seek for novel biomarkers that are useful in prevention of atherosclerotic diseases.

Objective: To improve the efficiency of dietary surveys, we reviewed the elements which have an affect on their reliability.
Methods: We evaluated the process phase of 12-day dietary record surveys (3 non-consecutive days in each of the 4 seasons).
Results: We have developed some accompanying PC tools (standardization coding manuals, e-learning system, mailing list, food coding errors search database, inquiry system for coding difficult cases, a structured feedback questionnaire for site staff). In conclusion, we were able to develop the tacit knowledge about dietary assessment that well-trained dietitians have, as several PC tools.

The aim of the project is to investigate association of cognitive ability with atherosclerosis-related factors. By the end of fiscal year of 2013 (Mar.2014), we were able to conduct examinations on more participants (751) than initially planned (400). ]
Preliminary analysis on 540 participants suggested an association of chronic kidney disease with cognitive decline, which we orally presented in an academic meeting held by the Japan Society of Cardiovascular Disease Prevention (June 15, 2013). We confirmed that there was no evidence of bias according to scorer of the cognitive function test. With further analyses combining more data, we will submit our final result to an academic journal soon.
