＜症例のポイント＞toxic shock syndrome(TSS)は黄色ブドウ球菌の産生するtoxic shock syndrome toxin-1(TSST-1)やエンテロトキシンが原因となって、高熱、全身性の皮膚の紅斑、ショック症状をおこし、多臓器不全をおこすブドウ球菌感染症の一種である。タンポンを使用していたこと、また口腔粘膜の充血と体幹・四肢にびまん性の境界不明瞭な紅潮を呈した特徴的な皮疹から、早期にTSSを疑い治療開始したことで救命しえた。患者の腟・タンポンの培養から検出されたStaphylococcus aureusはMSSAであり、TSST-1/SECの毒素産生株であったことが確認できた。(著者抄録)

© 2017 Japanese Society for Investigative Dermatology Background: Due to resistance and immune-related adverse events (irAE) some melanoma patients require ipilimumab after nivolumab therapy. However, little is known about the result of this switching. Objective: Investigate the outcome of ipilimumab switching in Japanese patients. Methods: We retrospectively collected 60 patients who were treated with ipilimumab after nivolumab from 9 institutes in Japan. Information of the primary tumor, treatment, response, irAE), and survival was collected. Results: In our cohort, acral lentiginous and mucosal melanoma accounted for 53% of the cases. The most common reason for initiating ipilimumab was disease progression (93%). Median interval from the last nivolumab administration to first ipilimumab administration was 29 days. Only 38% of patients completed 4 injections of ipilimumab. The best overall response was 3.6%. IrAE occurred in 78% of patients and 70% of those were of grade 3/4 (G3/4) and 31% of patients experienced 2 or more irAEs. An within interval of 28 days or less between the last nivolumab administration and ipilimumab administration was correlated with the development of G3

＜症例のポイント＞急速に全身にびらんが拡大したため、皮膚からメチシリン耐性ブドウ球菌(MRSA)が侵入し、敗血症を思わせる重症感染症を発症した尋常性天疱瘡(pemphigus vulgaris;PV)の重症例を報告した。発熱、倦怠感、全身のびらんの疼痛から、一時は危険な状態となったが、早期の適切な抗菌薬治療により感染症から回復した後は、天疱瘡の臨床症状スコア(PDAI)および血清中自己抗体価(ELISA値)は急速に改善した。自験例では、同時期に行われたステロイドパルス療法、免疫グロブリン大量療法(intravenous immunoglobulin;IVIG)が奏効した可能性は除外できないが、劇的に病勢のコントロールが得られ、退院後も再発・再燃の徴候がみられなかったことから、重症感染症に伴って自己免疫反応が減弱した可能性が考えられた。(著者抄録)

19歳,男性.2015年7月に左大腿内側に赤褐色の結節を自覚し,増大したため近医で単純切除された.発症前に外傷や下肢の浮腫はなかった.病理組織像では表皮直下から脂肪織にかけて核異型を伴う血管内皮様細胞が浸潤増殖し,一部に脈管形成を認めた.腫瘍細胞はCD31,D2-40が陽性でリンパ管マーカーで優位に染色された.PET-CTで術創部と左鼠径リンパ節に異常集積を認めた.前医での切除部残存病変から2cmの側方マージンで全摘出し,左鼠径リンパ節郭清術後,術後補助療法としてDG療法(ドセタキセル75mg/m2;第8日,ゲムシタビン1,000mg/m2;第1,8日)を3クール実施した.自験例は若年者の大腿部に発症した脈管肉腫で,代表的な病型のいずれにも当てはまらない非常に稀な症例であった.文献的考察により若年発症例では発症部位,基礎疾患,誘因,予後は多様であり,一定の傾向がないことがわかった.(著者抄録)

35歳,男性.Alport症候群による慢性腎不全に対して7年前に生体腎移植を受け,以後免疫抑制剤,ステロイドを内服していた.肛囲のコンジローマの多発による坐位保持困難を主訴に来院した.初診時,陰茎に幅3cmほどで帯状に,肛門周囲にはくるみ大の,臀裂周囲には手拳大の腫瘤が多発していた.臨床,病理組織像から巨大尖圭コンジローマと診断した.切除組織から外陰部に感染するヒトパピローマウイルスを網羅的に検索したところ,低リスク型の6,11型,高リスク型の52,59型を検出した.自験例は免疫抑制剤内服中であったことが尖圭コンジローマの巨大化,高リスク型HPVの感染の要因となったと考えた.悪性腫瘍との鑑別が重要であるが自験例では病理組織学的所見から区別しえた.(著者抄録)
